This form has been prepared in order to enable data subjects whose personal data are processed within the scope of Article 20 of the Constitution of the Republic of Turkey and Article 11 of the Law on the Protection of Personal Data to exercise their rights. <\/p>\n\n
As stated in the law, your applications must be in Turkish.<\/p>\n\n
Pursuant to the first paragraph of Article 13 of the Law, applications to be made to Keyfi Tat Kuruyemi\u015f, the data controller, regarding these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (“Board”).<\/p>\n\n
In this context, applications to be made to Ds Farma in ‘written’ form must be submitted to us physically, through a notary public or by registered e-mail using this form.<\/p>\n\n

Data owners who wish to apply electronically can send an e-mail to ds@dsfarma.com address of Ds Farma.<\/p>\n\n
In written applications, the date of notification of the document, and in other cases, the date of receipt of the application to the data controller shall be deemed as the date of notification. The applications made to Ds Farma will be answered free of charge “as soon as possible and within thirty days at the latest” from the date the request reaches Ds Farma, depending on the nature of the request, in accordance with paragraph 2 of Article 13 of the Law. <\/p>\n\n
Our responses to your applications will be delivered to you in writing or electronically in accordance with Article 13 of the Law. However, if the transaction requires an additional cost, we will charge the fee in the tariff determined by the Personal Data Protection Board. <\/p>\n\n
